MMM Healthcare, LLC. is an HMO POS plan and an HMO C-SNP plan with a Medicare contract. MMM Healthcare, LLC. is an HMO D-SNP plan with a Medicare contract and a contract with the Puerto Rico Medicaid program. Enrollment in MMM depends on contract renewal. To enroll, you must have Medicare Parts A and B and reside in one of the Island’s 78 towns. You must continue to pay your Medicare Part B monthly premium.
